For Title – please put your name (optional) and the name of your Addison’s dog.
For the Address/GPS section, add your town/city, state/province, and country. Please do not use GPS coordinates – it puts your marker in the middle of nowhere.
Use the Description section for info such as date (or year) of diagnosis, breed, recommended vet, medications, or whatever very brief comments you want to add.
You can also upload a picture of your dog!!!
